Your attorney representation may send you a signature authorization request through SMS (i.e. a text message).
1. You will receive a text notification from Medchart, which will ask you to proceed through a link that redirects to a webpage requesting you to electronically sign a consent form.
2. Once you click on the link, anew browser window on your mobile device will pop up that reads “Please provide electronic signature to <Name of Law Firm> to request records.” To provide your signature, click on “Draw Signature”.
3. Draw your signature in the space provided with your finger. If you are not satisfied with your signature, simply click “Clear” to restart. Otherwise, if you are content with your signature, click “Done”.
4. Next, you can draw your initials by clicking “Draw Initials.” Again, a window will pop up where the initials can be drawn. It can be cleared and redrawn by clicking “Clear.” Once satisfied, you may click “Done.”
5. Then click on the terms and conditions checkbox stated underneath your drawn signature and initials. This disclaimer gives consent to your attorney the authorization to affix a copy of your signature and initials on your behalf to request for medical records related to their representation of you. Click “Accept.”
6. Once you click “Accept”, you will be redirected to a page that outlines the three different consent forms that your electronic signature has been applied to. After scrolling through the3 pages, you will just need to click “Confirm”.
7. The next page will ask for photo identification – please kindly skip this step as your attorney will provide you with further instructions regarding photo identification.
8. You are now all set! Kindly wait to hear from your attorney representation for the next steps. You can now close the mobile browser.
